INJURY WAIVER AND GENERAL RELEASE FORM

I understand and acknowledge that by attending a North Pole Hoops event, I am exposed to a potential risk of body and person. I, hereby release North Pole Hoops, and any of its officers, directors, employees, agents, licensees, subsidiaries, consultants, independent contractors and affiliates from any and all liability for property damage, personal injuries or other claims arising from or in connection with his participation, including claims that are known or unknown, foreseeable or unforeseeable, future or contingent.

This release also provides North Pole Hoops the absolute, irrevocable, and unconditional right and permission with respect to the photographs/video that they have taken which may be included with others. North Pole Hoops has the ability to use, to re-use, publish and republish in whole or in part, individually or in conjunction with other photographs, in any medium (including but not limited to print, video or digital formats) and for any purpose whatsoever, including but not limited to: illustration, promotion, advertising and trade.

I agree that I will not now or at any time in the future, directly or indirectly, pursue, commence or prosecute any action, suit or other proceeding against North Pole Hoops and any of its officers, directors, employees, agents, licensees, subsidiaries, consultants, independent contractors and affiliates; Thus, any action, suit or proceeding arising out of or relating to the actions, causes of action, claims or demands are hereby waived, released and discharged by me. I agree, consent and affirm, to be bound by this Injury Waiver and General Release Form.

I acknowledge that I have read and fully understand the attached Injury Waiver and General Release Form. This agreement will be binding on me, my spouse, my children, my legal representatives and my heirs, successors and assigns.
